1. Please fill out this form if you're a new student of Studio Dance by Lina (i.e. first time to take classes even though you were previously a student, newcomer, etc)
2. Please fill out one form per enrollee. This page will automatically refresh and will allow you to submit another entry after clicking “Submit".
3. Remember to read through our policies page and to read all guidelines on the descriptions of our classes. To do so, go to our enroll page & click on your level or class.
4. Finally, all information submitted in this form will only be used for the official purposes of Studio Dance by Lina - which includes, but is not limited to - leveling purposes, special community events, and information dissemination. Your information will be kept secure and confidential, and will not be published or viewed by people outside of Studio Dance by Lina's employees and interns without your consent.
